As a Medical Director, you will:
Responsible for direction and oversight of the care and treatment to hospice patients rendered by the hospice care team and provide oversight for Plan of Care and all changes to the Plan of Care.
Implement the Plan of Care provisions to meet the patients medical needs.
Assess and reevaluate the patients medical needs on a regular basis.
Manage related expenditures in a fiscally responsible manner in accordance with the Companys budgets. Travel as necessary.
Abides by and demonstrates the company Mission Vision Values through both behavior and job performance on a day-to-day basis.
Consult and work with the patient's attending physician. Provide physician direction and guidance to the hospice program, any physician employees, and other staff and volunteers to assure the maintenance of quality care for patients and families.
Serve as a liaison and contact between the hospice program, community physicians and others engaged in health care services in order to develop and expand the number of patients served by Gentiva Health Services.
Responsible to ensure quality and participation of Associate Medical Director.
Provide direct medical care as needed to patient s in their homes, nursing homes and in the hospital.
Participate on the Professional Advisory Committee.
Provide input and physician oversight to Plan of Care and all changes to Plan of Care in coordination with the IDT and patients physician.
Attend Interdisciplinary Team meetings to review the Plan of Care and content of the active medical record.
Consult with patients' physicians and be available to members of the IDT.
Maintain liaison with the personal or attending physician. The attending physician is encouraged to provide primary care to his/her patient even though the patient also receives hospice care. May take over as attending physician, if primary physician requests.
Participate in CQI Committee meetings as asked.
Made regular contacts with practicing physicians to introduce the hospice program and to encourage participation in the palliative care hospice program.
Maintain clinical records, statistics, reports and records for purposes of evaluation and reporting of agency activities as prescribed and in compliance with local, state and federal laws.
Assist Executive Director and other supervisory hospice personnel in the planning, implementation and evaluation of in-service and continuing education programs. Assist in formulating, revising, implementing, and evaluating organization policies, procedures, goals and objectives.
Participate in public relation and community activities that promote the organizations role as an effective member of the Hospice care system. Promote customer service orientation to all hospice personnel.
Doctorate degree in medicine or osteopathy from an accredited medical school.
Current license to practice in the state.
Current DEA #.Knowledge of the principles and practice of primary medical care, with working knowledge of oncology medicine.
Hospice and Palliative Care certification preferred
Knowledge of hospice and palliative care with emphasis on control of symptoms associated with a terminal illness.
Knowledge of the local medical community and the ability to enhance the hospice's visibility and reputation with practicing physicians and hospitals.
Ability to work collaboratively with the patient's primary physician to interpret the hospice program and affect optimum medical care.
Ability to work within an interdisciplinary setting.
Understanding of Hospice philosophy, principles of death/dying.
Compliance with accepted professional standards and practices.
